Clinical notes

Notes a physio can finish on a phone, and the head physio can check.

A full Initial Evaluation on the first visit, a daily note that can start from the last one, and reassessments with the first scores on screen. Every note carries the name of the physio who wrote it.

The first visit, one box per part

A first visit opens as an Initial Evaluation. Each part has its own box, so nobody types the headings, and every note reads the same way whoever wrote it.

An approved Initial Evaluation, with its status, the objective assessment, the baseline outcome measures and the treatment plan
  1. Approved and locked. A note changed after approval is marked as edited and goes back for approval.
  2. The objective assessment, in its own box. The complaint, the clinical impression, the plan, the intervention, the frequency and the recommendation each have one too.
  3. Outcome measures recorded on the first visit. They become the baseline for every reassessment.
  4. The treatment plan, the intervention and the frequency. Once approved, the note prints on the clinic's letterhead.

Start from last visit

A new daily note can start from the last visit, or blank. Today's treatment and the plan come across ready to edit.

Last visit's subjective, objective and assessment sit under each box for reference. The physio copies one in only when it still applies, so last visit's findings end up in today's note only on purpose.

The admin chooses how this works for the clinic: offer it each time, always start from the last visit, or always start blank. Evaluations, reassessments and discharge notes always start blank.

Reassessment, with the first scores on top

Reassessments and progress reports use the evaluation layout. The first evaluation's scores sit at the top of the note, read-only, and today's go in below, so the change is on the screen when the physio talks it through with the patient.

The patient's earlier visits are listed beside the note, newest first.

The head physio approves, or sends it back

If the clinic wants it, every note goes to the head physio. They approve it, or send it back with a comment, and the physio finds it under My queue on their dashboard.

The queue filters by status and by physio, and a clinic with branches can look at one branch or all of them. The clinic can switch approval off, and then a submitted note is filed straight away.

Also in clinical notes

  • Seventeen outcome measures

LEFS, KOOS, Oxford Knee and Hip, WOMAC, Berg, TUG and the 6-minute walk test among them, on any visit. For licensed questionnaires the physio enters the score only.

  • Goals

Set at the first evaluation, updated at reassessment, and marked met or partly met at discharge.

  • Documents

X-rays, scan reports, blood work and referral letters on the patient's Documents tab. On a phone, Take photo opens the camera.

  • Home exercise programmes

Exercises from the clinic's library, with sets, reps, hold and frequency. The programme shows in the patient's app with videos, and can also go by email, print or WhatsApp.

  • Procedures performed (beta)

Physios log what they did on each visit without seeing a price. The desk bills it.

How it works

  1. 1. The patient is checked in

The visit moves onto the physio's My day list.

  2. 2. The physio opens the note

A first visit opens as an Initial Evaluation, a later one as a daily note.

  3. 3. They write it and submit it

On a phone or a computer, in the boxes for that kind of note.

  4. 4. The head physio checks it

Approved and locked, or sent back with a comment, if the clinic uses approval.
