Notes and carry-forward

How physios write the first evaluation, daily notes and reassessments, start a note from the last visit, and send notes for approval.

Physios write every note in Stator, on a phone or a computer. Each note has one box per section, so nobody types the headings.

Start the session

  1. Open the Dashboard. My day shows your patients for today and who has checked in.
  2. Open the appointment and click Start consultation. The visit opens with the patient's details at the top.

Feedback to address, also on the Dashboard, lists any note the head physio has sent back to you.

The first visit: an Initial Evaluation

A patient's first visit opens as an Initial Evaluation, with these sections:

  • Complaint
  • Objective assessment
  • Assessment / clinical impression
  • Treatment plan
  • Treatment intervention
  • Frequency
  • Recommendation
An approved Initial Evaluation note, with the complaint, objective assessment, outcome measures and treatment plan

A patient you've treated before on paper: in their first note in Stator, write a short history from the paper file. The main condition, when treatment started, the sessions so far, the current goals, and any allergies or precautions are enough.

Later visits: the daily note

The daily note has Subjective, Objective, Intervention today (one intervention per line), Assessment and Plan.

Reassessments and progress reports use the evaluation layout. A discharge note records the recommendation and the reason for discharge.

Start from last visit

A new daily note can start from the last visit, or blank. The admin chooses how this works under Settings → Compliance rules: offer it each time (recommended), always start from the last visit, or always start blank.

  1. Start the daily note and choose Start from last visit.
  2. Today's treatment and the plan come across ready to edit.
  3. Last visit's subjective, objective and assessment sit underneath each box for reference. Click Copy in only if it still applies.

Evaluations, reassessments and discharge notes always start blank.

Goals and outcome measures

  1. Goals: set them at the first evaluation, in the Goals panel on the visit. Update them at reassessment, and mark them met or partly met at discharge.
  2. Outcome measures: record any of seventeen on any visit, including LEFS, KOOS, Oxford Knee and Hip, WOMAC, Berg, TUG and the 6-minute walk test. For licensed questionnaires you enter the score only. At reassessment, the first evaluation's scores sit at the top of the note, next to today's.

Documents and home exercises

  • Documents: open the patient and go to the Documents tab to attach an X-ray, a scan report, blood work or a referral letter. On a phone, Take photo opens the camera.
  • Home exercises: open the patient, go to Home program, choose exercises from the library and click Prescribe program. Set the sets, reps, hold and frequency, and add a note for the patient. The programme shows in the patient's app with videos, and you can also send it by email, print it or share it on WhatsApp.

An approved note has Print / PDF, on the clinic's letterhead. Before approval, Preview PDF (draft) shows how it will look.

Procedures performed (beta)

On each visit, add the procedures you did under Procedures performed (beta). Physios don't see prices. The desk bills the visit separately, and physios never enter amounts or take payments.

Submit for approval

  1. When the note is complete, click Submit for approval.
  2. The head physio approves it, or clicks Request changes with a comment.
  3. A note sent back appears under Feedback to address on your Dashboard. Fix it and click Resubmit for approval.
  4. An approved note is locked. If it has to change after approval, Stator marks it as edited after approval, and it comes back for approval.

The clinic decides whether notes need approval, under Settings → Compliance rules. With approval off, submitting files the note straight away.

A note written late can count as an incident on the clinic's compliance report, if the clinic uses that rule.
